Becoming a patient
We've kept this simple on purpose. Here's how it will work when our doors open.
Join the interest list
Use the contact page to leave your name and phone number. When scheduling opens, we'll call in the order you signed up.
Book your first visit
Appointments can be made by phone or through our online patient portal once it launches. First visits are scheduled with extra time so we can actually get to know you.
Bring the basics
A photo ID, your insurance card, a list of current medications (bottles work great), and any recent records you have. Don't worry if you're missing something — we can request records from your previous provider.
Meet your provider
Your first appointment covers your history, current concerns, and a plan. You'll leave knowing what happens next — and how to reach us when you need us.
Patient portal & forms
New-patient forms and our secure patient portal will be available here before opening day, so you can complete paperwork from home. Check back, or join the interest list and we'll let you know.
