Capital City Family Medicine, LLC ("we," "us," or "the practice") is an independent family medicine practice in Frankfort, Kentucky. This policy covers our public website at capcityfm.com. Questions about it can be directed to the contact in Section 11.

Information you give us. If you email us or join our interest list, we receive what you choose to send — typically your name and phone number, and your email address. We ask that you send your name and contact details only, and that you keep health information out of these messages.

Information collected automatically. Like most websites, our hosting and security provider records basic technical information when you visit — such as your IP address, browser type, the pages you view, and the date and time. This is used to operate the site, keep it secure, and understand general traffic patterns. We do not use this to identify you personally.

3. Cookies and tracking

Our website does not set advertising or tracking cookies. We do not currently use third-party analytics that profile you across sites. If we add analytics in the future, we will update this policy first.

4. Third-party services

We rely on a small number of service providers to run the site:

• Website hosting and security (Cloudflare), which serves the site and protects it from abuse, and in doing so processes the basic technical information described above.
• Web fonts (Google Fonts), which loads the site's typefaces from Google's servers; this means Google may receive your IP address when the fonts load. We use this only to display the site.

These providers may only use the information to perform their services for us.

• To respond to your questions and phone or email inquiries.
• To contact you if you ask to join our new-patient interest list.
• To operate, maintain, secure, and improve the website.
• To comply with law and protect the rights and safety of the practice and the public.

6. This website is not for protected health information

This website and our general email are not secure channels for medical information and are not monitored for urgent needs. Please do not use them to describe symptoms, conditions, or other health details. Once we open, health information should be shared through your visit or our secure patient portal. 7. How we share information

We do not sell your information. We share it only with the service providers described above, or when required by law or to protect the safety and rights of the practice, our patients, or the public.

8. Data retention and security

We keep information collected through the website only as long as needed for the purposes above or as required by law, and we take reasonable measures to protect it. No method of transmission over the internet is completely secure, so we cannot guarantee absolute security.

9. Children's privacy

Our website is intended for a general adult audience and is not directed to children under 13, and we do not knowingly collect their information through the site. If you believe a child has provided information through the website, please contact us and we will delete it.

10. Changes to this policy

We may update this policy from time to time. When we do, we will revise the effective date above. Continued use of the website after a change means you accept the updated policy.
